“Penile Dermal Filler” Defined
The term “penis filler” can refer to many different techniques. For PhalloFILL®, it refers specifically to hyaluronic acid (HA) filler injected using a structured technique along the penile shaft to add girth. Unlike other filler substances, HA is not a foreign substance to your body. Your body produces and uses it in your joints, skin, and tissues to retain moisture. As an injection for penile growth, it acts as a volumizing agent. The filler adds physical thickness beneath the skin for a soft, natural appearance that’s reversible.

A PhalloFILL® appointment typically runs for about an hour from arrival to exit. Here’s what that time involves.
Your provider begins with a pre-procedure assessment. They review your anatomy, your target girth goal, and the number of units being placed that session. Most sessions involve four to six units.
Before any filler placement, the provider applies a local anesthetic using a fine, hair-like needle. This is the part men typically anticipate with the most anxiety. The majority of patients rate their pain experience as 1 or 2 on a 10-point scale. Men routinely express to us how surprised they are by how manageable it is.
Once the area is numb, the provider places the filler through a series of precise injections along the shaft. The technique is shallow. The filler is placed between the outer layers of penile tissue, above the erectile cylinders that control erection. PhalloFILL®’s protocol does not involve injecting into the blood vessels, nerves, or erectile structures responsible for erections. As a result, your ability to get and maintain an erection should remain unaffected.
After placement, the provider may gently shape the distribution to ensure even coverage. You leave the appointment with a visible increase in volume. However, swelling in the first one to two weeks can affect the initial appearance. The filler integrates with your natural tissue and settles during the initial period after treatment.

Some providers recommend avoiding over-the-counter NSAIDs for five to seven days before treatment to help reduce bruising. Always follow your provider’s instructions and consult the prescribing clinician before stopping any medication.
Healing and Long-Term Results
Understanding what happens to the filler following your session can help you set realistic expectations for the weeks and months ahead.
In the first 24 to 48 hours, some swelling and tenderness are normal. Bruising is also common and can appear more pronounced in this area than elsewhere on the body due to limited lymphatic drainage. The bruising sometimes travels down the shaft and can turn dark before clearing. This is expected and does not indicate a problem.
Over the first two to three weeks, the filler settles and integrates with the surrounding tissue. The initial post-procedure appearance changes during this period. What you see at week one is not what you’ll see at week three. Wait until full integration before evaluating your results.
After integration, the filler adds circumference in both the flaccid and erect states. The filler should not interfere with normal erectile expansion. The penis continues to enlarge during an erection as it did before treatment.
HA filler metabolizes gradually over time. Most men notice little change during the first year, and any reduction in circumference typically occurs slowly rather than all at once. Often, men choose to return after 12 to 18 months for maintenance treatments to help maintain their desired level of enhancement.
Sessions, Units, and Building Toward Your Goal
Most men aiming for a three-quarter- to one-inch increase in total girth require three to five sessions. Generally speaking, you’ll notice up to a quarter to a third of an inch per session. Sessions are spaced at least three weeks apart to allow full integration before additional filler is placed.
This staged approach is intentional. Placing too much filler at once increases the risk of migration and can reduce visible results. Building gradually gives your tissue time to adapt and allows both you and your provider to assess results at each stage before deciding on the next step.

Understanding the Procedure’s Limitations
Understanding what PhalloFILL® can accomplish is just as important as understanding what it isn’t designed to do.
PhalloFILL® is designed to increase girth (circumference) only. It does not increase penile length. Some men report an increase in flaccid length or a longer flaccid appearance because the added girth and weight can allow the penis to hang lower. However, this is a change in flaccid appearance, not an actual increase in erect length.
